Transitional Supportive Housing Eligible persons are homeless who live on the street, in an emergency shelter, in places not meant for human habitation, being evicted within a week, being discharged within a week from an institution in which the person has been a resident for more than 30 days, or fleeing a domestic violence situation. The Supportive Housing Program provides temporary housing/in-depth case management and supportive services to homeless persons or families while working with them to obtain permanent housing. The program also assists families in obtaining other community resources to help the families become self-sufficient. This program, through a grant by Kentucky Housing Corporation (KHC), can serve 40 families per year in our core county area. In addition, Foothills provides supportive housing services to 10 families per year in Franklin County. * Provides rental assistance (12 clients), case management, transportation, medical assistance, budget/credit classes, food assistance, heating/cooling assistance, utility assistance, educational assistance, GED preparation and referrals. Serves Clark, Estill, Franklin, Madison and Powell
Counties. Permanent Supportive Housing Eligible persons are homeless who live on the street, in an emergency shelter, in places not meant for human habitation, being evicted within a week, being discharged within a week from an institution in which the person has been a resident for more than 30 days, or fleeing a domestic violence situation and have a verifiable disability. The Permanent Supportive Housing Program provides permanent housing opportunities for disabled individuals or families. This program, through a grant by KHC, can serve ten families per year. * Rental assistance, case management, transportation, medical assistance,
budget/credit classes, food assistance, heating/cooling assistance, utility
assistance, educational assistance, GED preparation and referrals.
