"Your greatest wealth is health." Homeless people are often plagued with multiple health problems. Poor health frequently contributes to, or causes, a person or family’s homelessness. Several studies have found that one-third to one-half of homeless adults have some form of physical illness. The homeless population is at risk for trauma, drug and alcohol abuse, diseases related to exposure and infectious diseases such as tuberculosis. They are also at risk for chronic disease, including hypertension and heart disease, which are aggravated by neglect. HCH removes barriers to accessing care by bringing preventive comprehensive care into community programs that serve the homeless. This three-year grant will provide a service that will take medical care out of the traditional hospital-based setting and bring it to a population in need of health services. HCH addresses the isolated nature of Estill and Powell Counties, the distance between populated areas within rural counties, and the lack of transportation by providing health care that is mobile. HCH utilizes a mobile health clinic in conjunction with Riverview Medical Clinic in Estill County. Eligibility Requirements
Services Include:
Staff include a Licensed Nurse Practitioner, Registered Nurses and Case Managers. Medical staff provide services on Foothills Mobile Medical Unit that rotates between Estill and Powell Counties.
